Central Auditory Processing Dysfunction (CAPD)

Carol Atkins CAPD Assessment

Your appointment will include an interview, standard audiometric evaluation, stressed listening tests, and a review of the findings.

Please plan to spend two to three hours for this appointment. You are invited to observe your child being tested.

Please bring the following:

  • Completed Patient Information Sheet
  • Completed Central Auditory Processing Questionnaire
  • Completed Fisher Auditory Problems Checklist or Central Auditory Processing Disorders by Region
  • CHAPPS Checklist:
    • K – 6th grade please provide to the teacher/aid,
    • 7th – 12th grade please provide to the home room teacher
  • Copies of any hearing tests, speech-language pathology and/or psychology reports, or IEP notes, that have been done in the past 12 - 24 months for our records
  • A few toys so the child can entertain himself / herself while the parents are being interviewed by Mrs. Atkins and/or Dr. Granado.
